Comparative Study between Humby Knife and Shaving Blade Skin Grafts in 100 Patients: A Prospective Randomized Controlled Trial

Abstract: Background: Split-thickness skin grafting (STSG) is a cornerstone in reconstructive surgery. The Humby knife and shaving blade are two techniques for harvesting grafts, but their comparative efficacy remains understudied. This trial evaluates outcomes of both methods in 100 patients. Methods: A single-center RCT (2021–2023) 100 patients dividing 2 groups, having procedure of Humby knife (n=50) and shaving blade (n=50). Primary outcomes included graft take rate and complications; secondary outcomes were operative time, pain scores, and cosmetic results. Statistical analysis used t-tests, chi-square, and multivariate regression. Results: The Humby knife group had higher graft take rates (94% vs. 82%, p=0.03), lower infection rates (4% vs. 14%, p=0.04), and superior cosmesis (Humby : 6% vs. Shaving 18% (p=0.03%). Operative time was longer for Humby (28.5 ± 4.1 vs. 18.2 ± 3.8 minutes, p<0.001). Conclusion: The Humby knife offers superior graft survival and cosmetic outcomes despite longer operative time, advocating its use in critical reconstructive cases.

Hospital-Based Evaluation of Anaesthesia Practices in Bangladesh: A Retrospective Study

Abstract: Introduction: Anaesthesia management plays a critical role in ensuring patient safety and comfort during and after surgical procedures. The choice of anaesthesia type—whether regional or general—can significantly influence the incidence of complications and overall patient recovery. This study aimed to evaluate anaesthesia practices, monitor complications, and assess patient satisfaction among individuals undergoing various types of surgery. Methods: The study was conducted at General Hospital, Pabna, Bangladesh from 1st October 2023 to 30th September 2024. Data were collected from medical records of patients who underwent surgery at a hospital. The study population consisted of 42 individuals, with a higher proportion of females and the largest age group being 18-40 years. Data analysis was performed using SPSS version 26. Result: The results of this study indicate that 57.1% of patients received regional anaesthesia, while 42.9% were administered general anaesthesia. Monitoring techniques, including pulse oximetry, ECG, and blood pressure, were widely used, with complications occurring in 33.3% of the population. Post-naesthesia complications were most commonly nausea (42.9%) and hypotension (4.8%). Satisfaction with anaesthesia was high, with 95.2% of patients reporting satisfaction. A significant negative correlation (-0.361, p=0.0188) was found between general anaesthesia and the absence of complications. Conclusion: The findings from this study indicate that regional anaesthesia is associated with fewer post-anaesthesia complications compared to general anaesthesia, as evidenced by the negative correlation between the two. The statistically significant p-value of 0.0188 (p < 0.05) underscores the meaningful association between the type of anaesthesia and the occurrence of post-anaesthesia complications.

Endoscopic Management of Biliary Ascariasis

Abstract

Background:

Ascaris lumbricoides is common intestinal parasite and biliary ascaris is the
commonest extra intestinal manifestation of round worm. This study was aimed to review the
predisposing factors, presentation and the role of endoscopic management of biliary ascaris.

Method:

We performed retrospective analysis of 182 patients diagnosed and admitted as
biliary ascaris in the department of General Surgery, Bangabandhu Sheikh Mujib Medical
University, Dhaka from April 2016 and April 2021. Ultrasonographic findings of abdomen
were the mainstay of diagnosis.

Results:

Biliary ascariasis was more common in female (78%). The most common presentation
was pain (100%). ERCP was done successfully in 90.7%. All patients were given
anthelminthic. There was no complication.

Conclusion:

Ultrasound is a reliable screening test for diagnosis and ERC provides
confirmation. Once diagnosed, endoscopic extraction of the worm is often successful and
provides prompt relief to the patient. Antihelminthics should be administered for eradication
of worms from the intestine and to prevent their reentry into the biliary tree.

Elderly people at the Emergency Department: Study from a District Hospital in Bangladesh

Abstract Background: The proportion of elderly population is increasing globally. Very little information is available on the presentation and clinical outcome of elderly patients at the Emergency Department (ED) in Bangladesh. This study was carried out to assess the pattern of elderly patients (65 years and more) who visited the ED of 250 Bedded General Hospital, Pabna (GHP). Methods: A retrospective observational study was conducted on patients aged 65 years or more at GHP from 2017 through 2021. Patients’ demographic information, symptoms, diagnoses and outcome following attendance were studied. Statistical analyses were done using SPSS version 23. Results: 20.7% (n = 94,905) of all adults (n = 4,57,549) presenting to the ED were aged e” 65 years. A greater number of elderly patients (58%) required hospitalization than nonelderly adults (37%). The 3 most common symptoms were breathlessness (9.9%), dizziness (9.0%) and abdominal pain (5.3%). The symptoms were mainly acute (< 24 hours, 42%) or less than a week (28.9%). The 3 most common diagnoses were Respiratory tract infections (7.8%), Cerebro-vascular diseases (Stroke/Transient ischaemic attack, 7.6%) and heart failure (7.4%). Conclusion: Elderly people constituted 20.7% of emergency attendance but 32.5% of admissions in adults. They differ significantly from younger adults in presentation and outcome. A better understanding of this issue will help doctors at the ED provide better management.
